CCSD PK-12 Transportation Requests will now be included in Infinite Campus when you register for school classes. (Parents of K-12 Kuemper students: the transportation forms will be included in your school registration paperwork. )

• If NO Transportation is requested – please check NO.

• If Transportation IS requested – please complete the form included in registration on Infinite Campus.

• If your pick up/drop off location is a daycare – we need the name, address, and phone number for that daycare.
