The Iowa Athletic Association has updated the athletic participation form (sports physical form). CCSD will start to use it for the 23-24 school year.
Updated Form 🔗 https://www.iahsaa.org/wp-content/uploads/2023/03/2022-PPE-Form-Cover-3.13.23.pdf
Only PAGE 4 needs to be uploaded for athletic participation. Pages 1-3 will be completed and stored at the providers office or the health office at school.
Per the IAHSAA: "Due to HIPAA/FERPA regulations, a licensed health care professional and confidential storage of the sports physical form pages 1 through 3 is necessary, if those pages are to be kept at the school and used for medical purposes. Otherwise, pages 1 through 3 can be kept with the provider who performs the Preparticipation Examination and a waiver should be signed for release of information by the student athlete and parent if this is required by the school for participation of the student athlete. (Ref: 5th Edition of PreParticipation Physical Examination, 2019, pgs 25-27)".
The JH girls track meet in Carroll for tomorrow, April 21st has been canceled and will not be rescheduled due to conflicts. The JH boys track meet in Atlantic has been postponed to Monday, April 24th.

Meredith Parrott is a 7th grade student with autism. She created this “I Am” project for her language arts class. Parrott used her creativity to modify the project, instead of using white paper, Meredith turned hers into a colorful piece that symbolized a mason jar. Parrott stated, "I just wish people would understand.”
Thank you Meredith for sharing your story during Autism Acceptance Week.
